Post-acute or subacute care is a post hospital level of care designed to help patients’ recover from surgery, injury or serious illness. Montgomery Village Health Care Center provides the outcomes you need. We will help you reach your highest level of independence. Your individualized treatment plan may include pain management, therapeutic exercise for range of motion and strength, mobility training for safe movement in the home, adaptive equipment training and more.
